The purpose of this study was to examine (1) whether hospitals with greater strategic, technological and structural focus had better CABG outcomes than hospitals with lower levels of focus; (2) whether organizational focus affects outcomes beyond CABG volume effects; and (3) what organizational characteristics affect CABG outcomes. The conceptual model proposed the level of organizational focus would vary as organizations restructure to achieve the best fit with the environment and that the level of focus would affect outcomes. This approach necessarily required the development of a focus construct and its related dimensions: strategic, technological and structural focus.

A cross sectional design was used and data from the 2002 Health Care Utilization Project (HCUP) State Inpatient Database (SID) as the primary data source were utilized. The individual patient data were aggregated at the hospital level and assimilated with 2002 American Hospital Association (AHA) and 2002 Joint Commission of Accredited Healthcare Organization (JCAHO) data to incorporate hospital characteristic information.

Logistic and linear multiple regression methods were used to calculate expected mortality and length of stay (LOS), which were aggregated to the hospital level to determine the hospital-wide risk adjusted mortality and LOS ratios. A three-step, first and second order latent construct confirmatory factor analysis was utilized to derive the measurement model. Linear structural equation model (LISREL) analysis tested relationships between the measurement model and dependent and other variables.

The results supported the organizational focus construct model; however, the primary hypotheses relating focus to risk adjusted mortality and LOS were not significant. Several organizational characteristics were associated with CABG outcomes. CABG volume was not associated with CABG outcomes.

The study was the first to test a single and multi-dimensional focus construct in health care, where focus was defined using organizational strategic, technological, and structural variables. Knowledge of organizational factors affecting CABG patient outcomes might influence and shape important policies, which will ultimately affect the lives of people undergoing coronary artery bypass graft surgery.
