Partners in Diabetes (PID) represents a new initiative in healthcare that was designed and implemented through an active and democratic partnership between patients and providers in a primary care setting. The principal aim of this investigation was to analyze the process and evolution of this initiative in order to inform efforts in replicating comparable programs that are invested in improving the health of an identified community through active provider/patient partnerships. Using Action Research methodology and the Families and Democracy Model as a guiding conceptual framework, the study employed qualitative analyses of detailed PID meeting process notes (spanning 3 years) and in-depth interviews with PID participants. Key developmental themes across PID's course were delineated, as well as challenges that the program experienced. Future directions in research regarding the efficacy of PID as a supplement to standard diabetes care were proposed.
