This study evaluated the impact of a comprehensive tobacco cessation (TC) training program on pharmacists' TC counseling performances. Primary objective: to conduct exploratory research to study the effect a live training workshop has on pharmacists' ability to counsel patients to quit using tobacco. Secondary objective: to determine the relationship between pharmacists' perceived self-efficacy to provide TC counseling and their actual behavior performance.

Consistent with Self-Efficacy Theory, the continuing professional education (CPE) live training program incorporated exercises including modeling, rehearsal and feedback. The TC counseling process incorporated clinical practice guidelines, including the "5As," and application of the Transtheoretical Model (TTM). Pharmacists (N=24) participated in the CPE program and two simulated patient encounters pre- and post-training. The simulations varied: Scenario A (patient in the TTM action phase requiring extensive problem-solving skills); Scenario B (patient in the TTM preparation phase and requiring counseling guidesheet use to complete the "5As" process). Evaluation methods included videotaped patient simulations, surveys assessing pharmacists' counseling skills and confidence, and whether pharmacists provided TC services in their workplace. Client-Centered Model (CCM) of care items were also measured.

Pharmacists' TC counseling performance with simulated patients improved significantly post-training. Thirteen pharmacists provided TC services post-training. Several Scenario A performance measures were highly associated with pharmacists' service provision. Three Scenario B performance measures correlated with service providers' confidence to provide counseling. Pharmacists' self-efficacy was not significantly different between pharmacists who provided services post-training and those who did not, however, a relationship between self-efficacy and the practice environment where services were provided emerged. Application of TTM and CCM improved post-training.

A CPE program designed to include instructional strategies to enhance self-efficacy and counseling skills was created; this program increased pharmacists' ability to provide TC counseling during simulated patient encounters. Additional findings suggest the importance of incorporating various patient scenarios to assess performance, theoretical model application, and the likelihood of service provision post-training. These findings reinforce and are consistent with Self-Efficacy Theory which posits that higher self-efficacy can motivate practitioners to acquire and master skills necessary to perform proficiently. The effect practice environment has on confidence and service provision should be further studied.
