The purpose of this study was to explore the use of clinical journals to enhance development of reflective judgment in the first year of the clinical curriculum. The study also explored factors that impacted the development of reflective judgment.

Quantitative and qualitative assessments of reflective judgment were collected from a cohort of dental students.{09}Qualitative portions of the study used a case study approach by utilizing data collected from clinical journals and interviews (n = 16). The quantitative portion of the study used a one-group pretest-posttest design to assess changes in reflective judgment (n = 37) using the Reasoning about Current Issues (RCI) test.

Clinical journal and interview data were analyzed using a coding scheme based on the Reflective Judgment Model of Intellectual Development developed by King and Kitchener (1994). The Cronbach alpha was 0.76 for reliability of the coding scheme. A one-way repeated measure ANOVA demonstrated a positive change significant at the p < .05 level. Themes that had potential impact on development of reflective judgment emerged from the qualitative analyses and fell into two categories: interpersonal issues and curriculum issues. No particular theme independently demonstrated a negative impact on the average reflective judgment score.

The findings suggest no significant difference in pretest and posttest RCI scores between the control and intervention groups. However, there was a slight increase in the RCI score of the intervention group and a slight decrease in the RCI score for the control group.

The findings in this study provide dental administration and educators with a better understanding of the process of reflective judgment as it develops during the first year of clinical training in dental students. Critical thinking is receiving increasing attention in dental education, and this study provides some valuable insights as well as suggestions for further research.
