A mature and knowledgeable nursing staff is crucial to the success of any healthcare delivery organization. The nursing shortage threatens positive patient outcomes, decreases patient satisfaction, and potentially endangers patients. Stress is a leading cause of RNs leaving the profession, yet the effects of a nurse's personal trauma history on the subsequent development of compassion fatigue remain largely unexplored. The author drew a convenience sample of 107 registered nurses from an urban district of a state nursing association and from a nurse instructor, a nephrology nurse, and a clinical oncology nurse educator known to the author. All nurses were licensed in the state, had a minimum of one year's experience, and had primary duties of direct patient care. Participants completed an electronic survey, answering questions about personal demographics, exposure to potentially traumatic events, degree of distress caused by traumatic events, help experiences, available social support, burnout, and compassion fatigue. Statistical analysis of the data revealed no correlations between any individual predictors and the outcome variable (compassion fatigue). Multiple regression of the data revealed a three factor model of previous trauma, burnout, and general social support, which accounted for 55% of the variance in the study sample (R2 = .550, p = .001). Prior traumatization made the strongest contribution of the predictors (B = .890, p = .001). This model supports a positive association between prior traumatization and the development of CF in RNs. These findings may have important implications for individuals and organizations in the prevention and treatment of compassion fatigue.
