Pancreatic islet transplantation has emerged as a treatment option for Type I diabetics. Although the technique has proven feasible, there are still obstacles to overcome before islet transplantation can become a routine procedure. Studies designed to investigate strategies to improve pancreatic islet isolation and transplantation are presented in this thesis.

The optimal purification temperature in human islet isolation was investigated. Digested human pancreata were divided equally into two halves and then simultaneously purified at 4°C and at room temperature using a Cobe 2991 cell processor. The purified product was analyzed comparing the total islet number, the islet purity, and the degree of islet fragmentation. The condition of the islets proved to be better in the 4°C group with less fragmentation as compared to the room temperature group.

The ability of antioxidants to improve the engraftment of islets in rats was also investigated. Chemically induced diabetic rats underwent a syngeneic islet transplant and were randomly assigned to either a control group or to one of the three treatment groups (vitamin E, vitamin C, pyruvate). Individual treatments were given daily for two weeks during the perioperative period. Reversal of diabetes was then monitored. Results showed that pyruvate can significantly enhance the engraftment of pancreatic islets; however, the mechanism still remains unclear.

The induction of tolerance through the use of a monoclonal antibody against the transferrin receptor was also investigated. In a fully mismatched allogeneic mouse model, chemically induced diabetic recipient mice were randomly assigned to receive either an isotype control antibody or an anti-transferrin receptor monoclonal antibody for a short perioperative time. Tolerance specificity was tested with a skin graft from the same donor strain after islet transplantation without the use of immunosuppression. Results revealed that the anti-transferrin receptor monoclonal antibody promoted tolerance by islet graft acceptance, by evidence of reversal of diabetes and by the acceptance of the donor specific skin grafts without immunosuppression. The results of the described studies will help to advance the field of pancreatic islet transplantation towards becoming a routine treatment option for Type I diabetics.
