The purpose of this study was to develop and evaluate the effectiveness of a computer-tailored maintenance intervention designed to help participants improve or maintain positive dietary and physical activity behaviors, beliefs, and attitudes, and physiologic risk factors following participation in the NC WISEWOMAN program. Twenty-two North Carolina health departments were randomized to Maintenance Special Intervention (MSI) or Maintenance Usual Care (MUC). During the one-year program, MSI participants (n = 302) received six computer-tailored health mailings and two brief phone counseling calls from health department staff. Participants in MUC (n = 209) received usual health department follow-up services. At pre- and post-intervention trained staff from local health departments measured participants' serum cholesterol, blood pressure, and body weight. Phone surveys administered at pre- and post-intervention assessed participants' dietary and physical activity behaviors, beliefs, and attitudes, and acceptability of the intervention.

Among participants completing both surveys (n = 421), the majority began and ended the intervention in the maintenance stage of dietary and physical activity change, with high levels of self-efficacy, motivation, and social support. Intervention participants were more likely to move forward into later stages of physical activity change and increase dietary self-efficacy; control participants were more likely to increase dietary social support at follow-up. Both treatment groups maintained low levels of reported saturated fat and cholesterol intake, but less than optimal levels of physical activity at 12 months follow-up. Interpretation of physiologic risk factor change analysis was severely limited due to a low return rate (n = 98). Nevertheless, both treatment groups showed improvements in total and HDL cholesterol at follow-up. There was little change in blood pressure or body weight in either treatment group. Participants reported a high level of satisfaction with the maintenance intervention in terms of personal relevance, perceived effectiveness, and ease of use.

The study findings suggest that women in both groups were able to maintain positive dietary behaviors, and health beliefs and attitudes for up to one year following the initial intervention program. Due to substantial loss to follow-up for physiologic risk measures, a more controlled trial of the Maintenance intervention is needed to determine its true effectiveness in modifying CVD risk status.
