This manuscript reports the results of a study that investigated whether the outcomes, body weight and physical function, improved when elderly residents received enteral nutrition in two long-term care facilities, the Amsterdam House and Isabella Geriatric Care Center. The study further investigated whether clinical indicators of the quality of enteral nutrition---adequate feeding and gastrointestinal complications---were associated with increased body weight, improved physical function and an additional outcome, decreased incidence of infections. The study also investigated potential relationships between age, gender, nursing home site, co-morbidities, reason for enteral nutrition, treatment with physical therapy, and each of the outcomes to control for variables other than the clinical indicators that might have had significant effects on the outcomes.

The medical records of 67 residents were retrospectively reviewed over a 3-month period starting with the initiation of enteral nutrition. Information obtained from medical records was coded into a computerized spreadsheet and then imported into a computerized statistical program to analyze the data. The study was granted exempt status by the institutional review board of New York University and by the Weill Medical College of Cornell University.

Body weight increased significantly (p = 0.008) and physical function improved significantly (p = 0.0007) after the initiation of enteral nutrition. Adequate feeding (r = 0.211, p = 0.0001) and absence of gastrointestinal complications (r = 0.286, p = 0.01) were significantly associated with increased body weight as analyzed by multiple regressions. Among the variables age, gender, nursing home site, co-morbidities, reason for enteral nutrition, and treatment with physical therapy, only treatment with physical therapy was significantly associated with improvement in physical function (p = 0.01) as analyzed by Wilcoxon rank-sum.

The findings suggest that adequate feeding with enteral nutrition and absence of gastrointestinal complications were associated with increased body weight. Physical therapy, rather than enteral nutrition was the probable reason for improved physical function. Gastrointestinal complications while on enteral nutrition did not pose a risk to residents that would preclude the use of enteral nutrition.

The practical implications of these results are that enteral nutrition is indicated for elderly residents of long-term care facilities who are also candidates for physical therapy. Future research should verify these results in larger populations and investigate additional outcomes to provide additional evidence of the effectiveness of enteral nutrition in residents who are permanently immobile, i.e. not candidates for physical therapy.
