Danshen (Salviae miltiorrhizae) is a Chinese herb that has been used for thousands of years to treat cardiovascular and neurological diseases. However, the mechanisms by which danshen exerts its therapeutic effects are not known. Four approaches were launched to determine whether and how danshen supplement is beneficial. Methods. A literature review indicates that danshen or its active ingredient, tanshinone IIA (TIIA), may possess antioxidation activity, inhibit coagulation and inflammation, and reduce cancer cell growth. An in vitro study was conducted to investigate whether TIIA reduces apoptosis induced by reactive oxygen species (ROS) and cholesterol overload in J774 mouse macrophages. Gene profiling using microarray technology was used to identify gene patterns affected by TIIA in PC-12 rat pheochromocytoma cells. An in vivo study using apoE deficient (apoE -/-) mice fed with TIIA was performed to determine whether the formation of atherosclerosis was affected by TIIA treatment. Results. TIIA significantly inhibited the cell death induced by hydrogen peroxide in the J774 macrophage cell line and the protective effect was comparable to the positive control, trolox. TIIA also inhibited cholesterol-induced cell death in J774 cells treated with acetylated low density lipoprotein (AcLDL). Gene expression in PC-12 cells treated with 3muM TIIA overnight were assessed by two microarray platforms and results indicated that TIIA affected expression in genes involving response to ROS, cell cycle, cell growth and apoptosis. However, the size of atherosclerotic plaques, plasma cholesterol and triglyceride levels in apoE -/- mice receiving 50mg/kg TIIA via gastric gavage daily for 21 days was not different from control mice. Conclusion. Although the literature survey, the in vitro experiments and gene profiling results point to potential beneficial effects of danshen and TIIA in disease protection, TIIA did not manifest similar effects in the in vivo model. TIIA may not be the active ingredient in danshen extract. Overall, the efficacy of danshen supplement is not clear and it should not be used as the primary treatment.
