Aim. The literature suggests that a conceptual similarity exists between somatization disorder and alexithymia in the area of cognitive affective difficulties in identifying and describing feelings. This study utilized a pretest-posttest design to measure alexithymia (utilizing the Twenty-Item Toronto Alexithymia Scale [TAS-20]) and somatization disorder (utilizing the Othmer Somatization Screening Test and the Wahler Physical Symptoms Inventory [WPSI]) screening test score changes in a randomly selected sample of patients with higher than average medical services utilization rates prior to and following a 12-week cognitive-behavioral group intervention.

Methodology. 50 adult members (35 women, 15 men), ranging in age from 29 to 83 years (mean = 51.9; s.d. = 13.4) of a large Northern California health maintenance organization were recruited from the HMO's data bank of utilization rates and from physician, registered nurse, and social worker referrals. Each of the six group therapy interventions in five HMO facilities was conducted by a licensed health professional (physician, psychologist, or social worker) and a somatics therapist.

Results. The 34.7% rate of alexithymia in the subject sample was consistent with other studies examining the relationship of alexithymia and physical symptoms. Somatization screening scores rates ranged from 83.7% of subjects scoring above the cut-off on the Othmer to 98% of subjects scoring above the cut-off on the WPSI.

Statistically significant pretreatment to posttreatment TAS-20 score change reductions were found in the non-alexithymia stratum (p = .036), suggesting that the treatment intervention enhanced the identification and description of feelings in subjects who had the capacity to do so.

The lack of association between TAS-20 total scores and the number of physical symptoms reported (61% of subjects scoring in the alexithymic strata and 61.5% of subjects scoring in the non-alexithymic strata screened positively for somatization disorder) support previous findings that (1) alexithymia scores are unrelated to somatoform disorders and (2) that alexithymia and somatization reflect separate constructs that may occur simultaneously.
