Supportive models of augmented care in teen pregnancy have been explored in the literature, however no studies have examined the community doula model utilizing trained para-professionals of a same culture. Few studies have examined how pregnant and parenting teens perceive social support and hope for future. The primary purpose of this study was to explore and describe the perception of social support and hope in twelve pregnant and twelve parenting teens who had received support from a doula during pregnancy, labor and delivery, and post-delivery. Results from this study suggested that the doula provided unique components of care including: dissemination of knowledge in the teen's context; emotional and physical support during pregnancy and labor; advocacy for self; and assistance with maturation. Doulas enhanced the teen's attachment to the newborn, breastfeeding initiation, and education related to newborn safety issues. Findings also suggested that teen mothers relied more on family as network support during pregnancy than following delivery, although what a teen wanted from family, such as assistance with basic needs, was not always available to her. Teen's responses about hope for future, despite an impoverished past, included: positive reflections for a brighter future; recognition of challenges to overcome the social conditions in which they reside; and the need of adult assistance to attain goals.

Broadening nurse's understanding of the role of a doula in maternity care may reduce tension with nurses who view the doula as a replacement of their role and threat to the traditional labor and delivery staff nurse position. Additionally, hope engendering strategies nurses can incorporate into teen maternity clinics or childbirth education classes beyond the traditional nursing role include: posing question's about future goal orientation during pregnancy; providing assistance in initiating the process toward goal-achievement; and providing encouragement to take steps toward goals.

Implications for future research include: the need for additional studies surrounding the benefits and positive causal relationships between doula care and teen pregnancy; further exploration regarding specific characteristics of community based models of social support with para-professionals of same culture; and further exploration of how hope-engendering strategies can impact brighter futures for young families.
