The existing literature provides some empirical studies on the resilience of mental health safety net institutions in rural areas, specifically their resilience in response to a rural health environment and managed care. But the limitation of these studies is the lack of a coherent sociological theory to explain the resiliency of safety net institutions. This dissertation seeks to fill this gap in the literature and also to answer the empirical research question, "What specific characteristics of rural mental health safety net institutions foster resiliency?"

To help answer my general research question, I will examine how three distinct safety net institutions in rural New Mexico remain resilient in response to a rural setting and a Medicaid managed care environment. What do safety net institutions do to manage delivery of mental health care services? I adapt Marxist literature to explain the role of the state in relation to the resiliency of safety net institutions. I also draw from institutional theory to demonstrate the organizational processes in which safety net institutions attempt to remain resilient while contending with the complexities of a rural setting as well as a Medicaid managed care environment.

I use data collected from July 2002 to 2004. A total of ninety interviews were conducted with mental health providers, staff, and clients at three safety net institutions. My findings indicate that safety net institutions, in response to a rural setting and a Medicaid managed care environment, employ strategies such as downsizing or eliminating programs, partnering with other safety net institutions, and streamlining. Another key finding shows that safety net institutions do not operate independently of each other. Rather, interagency networking exists where safety net institutions possess formal and informal networks with other safety net institutions and social service agencies. Interagency networking contributes to the resiliency of these safety net institutions and allows safety net institutions to negotiate the complicated terrain of providing mental health care services in a rural setting, while also trying to negotiate the Medicaid managed care terrain.

Future research studies need to consider the complexity of delivering services in a rural setting and the practice of health care policy in rural areas. Policymakers should account for the various characteristics of resiliency such as interagency networking in rural settings when implementing health care policy. Consistent monitoring and evaluation of state actions like the Medicaid managed care program in rural areas is critical.
