Primary care providers (PCPs) play a critical role in the identification and treatment of youth with psychosocial problems. Much research has suggested that many youth who have even serious mental health problems are not identified during primary care visits and do not receive appropriate treatment. PCPs typically under-identify the majority of youth with psychosocial problems and many parents report that they do not discuss their child's psychosocial problems with the PCP.

This dissertation investigates the discussion of psychosocial issues during primary care visits and PCP identification of psychosocial problems. The characteristics of youth examined in this dissertation include mental health symptoms, functional impairment, burden to the family, physical pain, age, and gender. Family characteristics include race, ethnicity, and poverty. Finally, PCP characteristics include gender, attitudes and burden associated with treating psychosocial problems, confidence in psychosocial treatment and referral skills, job control and satisfaction, and the accessibility of mental health specialists.

Results. The discussion of psychosocial issues was more common when youth demonstrated hyperactivity symptoms but was 48-51% less likely when youth demonstrated physical pain, even during visits with youth who were impaired by their mental health problems. The findings provide strong support that psychosocial issues do not receive adequate attention during primary care visits when physical problems are also present. The discussion of psychosocial issues was also more common with female PCPs and with those PCPs who demonstrated greater confidence in their psychosocial treatment skills.

PCP identification of psychosocial problems was related to the both internalizing and externalizing symptoms as well as functional impairment and burden to the family. Youth were most likely to be identified as having a problem when they were impaired and demonstrated externalizing symptoms. PCP identification of psychosocial problems was also more likely when the parent reported discussing psychosocial issues with the PCP and when the PCP reported feeling less burdened by treating psychosocial problems. PCPs who felt more burdened were less likely to identify youth with mental health problems. PCPs who reported easier access to mental health specialists were less likely to identify youth as having a psychosocial problem.

Future longitudinal research should examine the complex pathway between the development of symptoms, recognition of a problem, and use of services. Interventions should target decreasing PCP burden and equipping PCPs with the skills and confidence necessary to gather and use psychosocial information to improve the well-being of youth and their families.
