Introduction. Bipolar disorder among youth is an important public health concern because it leads to great disability as evidenced by high rates of morbidity and mortality and a high level of service use. However, there is a great paucity of information on the degree of burden to the healthcare system and patterns of service use relative to youth with other behavioral health disorders and the variation in need among youth diagnosed with bipolar disorder. Therefore, this study will use the 2000-2001 Thomson/Medstat-MarketScan RTM database to assess patterns of health services used, treatments prescribed, and diagnoses received in children and adolescents diagnosed with bipolar disorder and compared to youth with other behavioral health disorders.

Aims/Methods. The goals of this research are to (1) prospectively assess the restrictiveness, complexity and continuity of mental health service use and treatments received among children and adolescents with bipolar disorder, adjusting for differential risk associated with mental and medical comorbidities; (2) examine adherence to the current practice guidelines and their association with service use; and (3) to retrospectively assess the longitudinal patterns of psychiatric diagnoses, medical comorbidities, mental health treatment, and service utilization for a cohort of children and adolescents before they receive a bipolar disorder diagnosis and to examine how patterns of treatment change after diagnosis.

Contribution and significance. Overall, this study improves the understanding of the patterns of service use and the type of care provided for children and adolescents prior to and following their diagnosis of bipolar disorder. Using this information, we can construct a comprehensive picture of these youth's resource utilization. Results of this study will provide important information for developing a model of care that can more adequately address the unique needs of these patients and lead to improved approaches for treatment and service delivery.
