This study investigated the relationship between type of substance abuse (alcohol/illicit drugs versus prescription drugs) and type of referral for drug treatment (occupational versus nonoccupational) among health care professionals. A review of the literature revealed that prescription drug abuse and illicit drug/alcohol abuse by health care professionals are on the rise, and that identifying both types of abuse in the workplace is often ignored until a crisis occurs. Furthermore, the dynamics of the two types of abuse are different, which in turn creates differences in how abusers are referred for treatment: whether from occupational (e.g., colleagues, supervisors) or nonoccupational (e.g., therapist, family member, legal system) sources. Data were obtained from 200 surveys completed in the Illinois Health Professional program by health care professionals admitted during 2002--2005. Chi-square analysis was used to test the primary hypothesis that differences exist in the referral source according to the type of drug used. Results failed to support the primary hypothesis and showed no difference in type of referral by type of drug abused. Logistic regression was used to determine psychosocial and health predictors of type of drug abuse (prescription drug versus alcohol/illicit drug). Results of the logistic regression analysis indicated that (a) poorer general health was associated with alcohol/illicit drug use, (b) work limitations were associated with prescription drug abuse, and (c) experience of pain was associated with prescription drug abuse. This study can contribute to social change by highlighting the importance of timely occupational and nonoccupational referrals for health care professionals who abuse alcohol and prescription drugs. Additionally, correlates of different types of abuse may contribute to enhancing the referral process, thereby improving the delivery of health care services.
