Computer aided interpretation describes the use of computer algorithms to facilitate the interpretation of medical images. In this dissertation, I present novel algorithms for computer aided interpretation in three categories: (1) data modeling, (2) volumetric visualization, and (3) computer aided detection (CAD). I evaluate these algorithms on a number of clinical applications, but primarily on CT colonography (virtual colonoscopy).

In the area of data modeling, I present a new algorithm for computing the 3D medial axis transform to find the centerline of various anatomic structures. I demonstrate its utility in automatic navigation of image volumes and quantitative analysis of anatomic structures. The evaluation includes several different applications: CT colonography, carotid artery stenosis detection from 4D ultrasound, and abdominal aortic stent graft planning. I also present novel evaluation methods for manual and automated lesion detection.

In the area of volumetric visualization, I present a number of novel visualization modes, with emphasis on a new visualization paradigm that I call map projection visualization. I empirically demonstrate how this paradigm solves the problems of conventional visualization and evaluate it for the detection of polyps from CT colonography.

In the area of CAD, I present two novel algorithms: the surface normal overlap algorithm and the lantern transform. These algorithms are both used for the detection of colonic polyps from CT colonography and for the detection of lung nodules from chest CT. I evaluate the accuracy of these algorithms as well as the gain in performance when radiologists utilize these algorithms.

Computer algorithms that analyze medical images have been described for many decades, but it is only recently that the need for computer aided interpretation has become irrefutable. This dissertation presents novel computer aided interpretation algorithms and demonstrates how they can be used to solve the information overload problem in several clinical imaging applications, with emphasis on CT colonography.
