Three-dimensional ultrasound (3D US) provides proven qualitative (increased understanding of complex geometric structures) and quantitative benefits (ability to accurately measure volumes) over standard two-dimensional (2D) US techniques. However, the clinical use of 3D US has been limited due to its cost and performance limitations, and lack of clinical applications that make 3D essential. In this research, we investigate both of these issues.

First, we have developed a low-cost high-performance PC-based 3D US system. This system uses a modern multimedia processor and optimized reconstruction and visualization algorithms to provide a clinically usable system. Reconstruction of 512 x 512 acquired images occurs at over 10 frames/s and allows incremental reconstruction so that the user can visualize volumes "grow" during 3D scanning. A simple Maximum Intensity Projection (MIP) visualization algorithm allows users to interactively rotate 128 x 128 x 128 volumes at 10 frames/sec.

Second, we have used our 3D system to measure placental volumes in a pilot clinical study. The results of this study indicate that there is a strong correlation between placental volumes computed using our system and fetal birth weight and suggests that this metric could predict fetal outcome. Additionally, we have developed a new scanning method to image large volumes with our 3D system as many placentas were too large to fit within the standard field of view of the ultrasound scanner.
