The purpose of this study was to examine the stressors and stress levels, loneliness, and depression experienced by Taiwanese rural elders and to determine if loneliness, perceived health status, and a variety of demographic characteristics are precursors to stress. This study also sought to explore the relationship between stress and depression. A cross-sectional design utilizing face-to-face administration of instruments was used. A total of 201 older rural community residents participated in the study. Stressors and stress level were measured by the revised Taiwanese Elderly Stress Inventory, perceived health status was measured by the Health Perception Scale, loneliness was assessed by the UCLA Loneliness Scale-Version 3, level of depression was assessed by the Geriatric Depression Scale-short form and the Apparent Emotional Rating Scale. Descriptive statistics, t-tests/ANOVA, Pearson correlation, multiple regression, and Chi-square were performed for data analysis.

"Decrease in individual physical strength or tolerance" and "Having grandchildren or great-grandchildren" were the stressors mostly experienced by Taiwanese rural elders while "Bad habits of family members" and "Divorce or separation of your children" had the highest mean score of stressfulness. The mean score of stress level was 39.61 (SD = 23.7), indicating higher stress level experienced by rural elders when compared with a study conducted in urban elders in Taiwan. The mean score of loneliness was 43.59 (SD = 9.30), indicating a moderate loneliness level. In this study, differences of stressfulness were found between demographic variables of sex, educational level, living arrangement, and SES. Loneliness, perceived health status, and education were factors that significantly contributed to the stress level (r = 3.95; -3.84; -2.69; p ≤ .01 to .001). Overall, 56% of the subjects were depressed. Also, significant correlations were found between depression and a number of stressors ( r = 6.65--26.5; p ≤ .001). The findings of this study will provide health professionals in Taiwan with a better understanding and recognition about stress and stress related-health problems in rural elders and assist community health policy-makers in determining the need for health-related services for rural elders experiencing these problems.
