Funding pressures and market pressures require military hospitals, referred to as military treatment facilities (MTFs), to operate at the greatest efficiencies possible. Organizational alignment theory maintains that institutional alignment of strategic planning, organizational design, staff integration and stakeholder expectations lead to increase the efficiencies and effectiveness of organizations. This study determined if there was a correlation between organizational alignment in MTFs and high performance reviews as determined by the Navy medical inspector general (MEDIG). The study also showed statistical evidence on which organizational alignment characteristic has the greatest impact on higher MEDIG evaluation scores.

Organizational alignment is defined as the degree to which all elements of an organization are closely integrated and share common understandings of the organizations strengths and weaknesses. All parts of the organization must work in concert and be supportive of each other to enable the organization to achieve high performance results. Organizational alignment is an ongoing process that requires continuous monitoring and adjustments. Continuous checking and realignment are essential for change and positive results. Alignment is not only a concept pursued by management, but must be accepted and applied by staff throughout the organization. However, organizational alignment must be embraced by senior management, who lead the organizational culture and climate changes that result in high employee moral, loyalty and dedication.

Commanding Officers (COs) of MTFs come from a variety of professional backgrounds and levels of management experience. Their seniority and rank underline their leadership skills and pre-command management courses attempt to provide the necessary skills for them to successfully lead their hospitals. However, the military's rank structure and bureaucratic structure traditionally does not support the concepts of organizational alignment. The classical military organization is built on a hierarchy of controls, and communication/information flow based on divisions of labor, which could undermine a staffs' ability to work in concert and limit performance and growth. The concept of alignment as practiced by Wal-Mart, Dell Computers, Federal Express, Toyota, Honda and Nissan has lead to unparalleled performance results. COs are under increasing pressure to improve MTF productivity and performance and may benefit from applying organizational alignment theory within their commands.

The study analyzed any correlation between those commands that apply concepts of organizational alignment and improved performance as determined by MEDIG evaluations. The study has practical applications by not only exposing alignment theory to Navy medical leadership, but by highlighting its positive results in MTFs and identifying which alignment concepts lead to the greater performance outcomes at MTFs.

The research findings showed a weak correlation between MTF organizational alignment and high MEDIG scores. The correlation is positive, but weaker than anticipated, since Navy medicine has all the elements in place for its MTFs to be highly organizationally aligned and become high performance organizations.
