Advanced paternal age (APA) is not well defined as it relates to genetic counseling practice. Both the ACMG and ACOG indicate that only individualized counseling for APA is appropriate, however without specific guidelines establishing counseling criteria, it reasons that considerable genetic counseling variation must exist regarding APA. A literature review and a study of current genetic counseling practices regarding APA were conducted. Results of the study confirmed the hypothesis that significant variation exists in genetic counseling for APA in terms of ages cited and associated complications. Major obstacles potentially leading to the diversity in practice are discussed. Sex chromosome anueploidy, 47, XXY specifically, may warrant further attention, as relatively higher rates of Klinefelter syndrome are linked to APA, and this may have clinical implications for prenatal diagnosis. These results demonstrate both the need for consistency in genetic counseling practice regarding APA as well as the need for further research regarding the associated effects of APA on pregnancy.Subjects--Topical Terms:
