This thesis describes a series of 10 studies conducted from 1996 to 2001 that examined the epidemiology of cervical human papillomavirus (HPV) infection in Hong Kong. HPVs play a major role in the development of cervical cancer. The age-standardised incidence of cervical cancer in Hong Kong remains between 10 to 15 per 100,000, which is moderately high compared to other industrialised countries. A thorough understanding of the epidemiology of cervical HPV infection would further improve the control of cervical cancer in Hong Kong as well as other countries.

The first four studies were cross-sectional surveys on HPV prevalence and genotype distribution among sexually transmitted disease clinic patients, pregnant women, colposcopy clinic patients, and cervical cancer patients. The most striking finding of these studies was the high prevalence of HPV58, a worldwide rare genotype, in all groups of women examined.

The sixth and seventh studies examined the oncogenic risk implications of intratypic variants of the two most prevalent genotypes, HPV16 and HPV58. The epidemiology and risk implications of HPV16 variant infection in Hong Kong were markedly different from other countries. An HPV58 variant, E7 T201 G63S, was found to have a higher oncogenic risk.

The eighth and ninth studies assessed the role of beta- and gamma-herpesviruses as co-factors for the development of cervical neoplasia. The last study was a two-year prospective study, which showed that HPV testing at the first visit was superior to regular follow-up using cervical smears in predicting/detecting progression of undetermined and low-grade cervical cytological abnormalities. The high negative predictive value of HPV test could potentially decrease unnecessary follow-up visits.
