The majority of adults in the United States affected by mental illness are, or will become, parents (Nicholson et al., 1999). Children of parents with mental illness are known to be at-risk for psychological problems (Dickstein et al., 1998). Parents with mental illness say that not being able to parent their children compromises their well-being, and impedes their progress towards recovery (Blanch et al., 1994). In 1990, most state mental health agencies (SMHAs) were not addressing parenting as a normal, primary life role (Nicholson et al., 1993).

This study has three aims: To determine whether SMHAs are responsive to the parental role of adult clients; To determine the relationship between contextual characteristics of states and whether SMHAs respond to the parental role of adult clients; and, To explore factors related to SMHAs' responsiveness to the parental role of adult clients. Primary data include (a) a 1999 national survey of 51 SMHAs' responsiveness to the parental roles of adult clients; and, (b) telephone interviews with 25 stakeholders in four states. Secondary databases include (c) the National Association of State Mental Health Program Directors individual state profiles; (d) the United States Census Bureau individual state profiles; and, (e) an index of fiscal comfort.

Results suggest that approximately half of SMHAs are responsive to the parental role of adult clients, while few SMHAs have specific programs or services for parents and even fewer have policies or guidelines to address the unique needs and experiences of parents and their families. Independent SMHAs and SMHAs serving only mental health populations are significant predictors of SMHAs' responsiveness to the parental role of adult clients. SMHAs' organizational philosophy, collaboration within SMHAs and of SMHAs with other service systems, and specialized advocacy efforts are significant contributors to SMHAs' responsiveness to adult clients in their roles as parents.

Opportunities to support families experiencing parental mental illness are often missed, resulting in compromised outcomes for parents, children, and families. SMHAs need to assume a leadership role, and prioritize identifying, and responding to, the parental role of adult clients.
