The general reticence of practitioners to deliver smoking cessation interventions is a critical component of the public health problem of smoking. Despite widespread knowledge about the deleterious effects of smoking on health, many clinicians fail to address the issue with their patients who smoke. Understanding the influence of both patient and practitioner motivation on patients' smoking outcomes is, therefore, integral to intervening more effectively with people who smoke.

The theoretical model for this study was self-determination theory (SDT). The design also was informed by Gadow's moral framework for nursing, existential advocacy. The study purpose was to explore the influence of patients' perceptions of their practitioners' motivation for delivering smoking cessation intervention on the motivational processes that underlie smoking cessation. In addition to testing the SDT model for smoking cessation, construct validity of the Perceived Practitioner Motivation Scale (PPMS) was explored.

A descriptive, correlational design was used and the study was nested within a larger randomized clinical trial designed to test an SDT-based intervention with people who smoke. Participants were 166 adults who were recruited consecutively and were assigned to any of the groups in the larger study.

Perceived practitioner motivation was measured using the PPMS, autonomy orientation with the General Causality Orientation Scale (autonomy subscale), autonomy support with the Health Care Climate Questionnaire (modified version), autonomous motivation by the Treatment Self-Regulation Questionnaire (autonomous reasons subscale), and perceived competence with the Perceived Competence Scale. Measures were obtained at baseline and 1 month. Hypotheses were tested using regression analyses. Interviews also were conducted with 10 purposively sampled participants, based on their PPMS scores.

Perceived practitioner motivation moderated the relationship between autonomy support and autonomous motivation as well as between autonomy support and perceived competence. Consistent with the tenets of existential advocacy, both of these relationships were strongest in the presence of a practitioner who was experienced as highly motivated for delivering smoking cessation intervention. Perceived practitioner motivation did not moderate the relationship between autonomy orientation and autonomous motivation. Support for the reliability and validity of the PPMS was demonstrated. The SDT model for smoking cessation was generally supported as well.
