The present study used a key informant survey to assess key community stakeholders' knowledge and perceptions of psychiatric advance directives in two Ohio counties-one that was involved in a pilot psychiatric advance directive education project and one that has yet to begin an education initiative. The key informants included legal and law enforcement personnel, clergy, healthcare professionals, and mental health professionals, consumers, and family members. A questionnaire was developed to assess perceptions about psychiatric advance directives, the Opinions about Psychiatric Advance Directives (OPAD) questionnaire. Two hundred and sixty-two key informants completed the OPAD, demographic questions, questions about familiarity with psychiatric advance directives, and provided qualitative comments on the documents. Principal components factor analysis of the OPAD revealed four primary factors: <italic>Obstacles to Success, Utility and Benefits, Essential Professionalism</italic>, and <italic>Self-Determination </italic>. Results indicate that although most of the key informants had never heard of psychiatric advance directives, they generally believed that the documents could positively impact mental health treatment. The results also suggest that despite having relatively positive views, the key informants had considerable concerns about the implementation of psychiatric advance directives. Mental health informants, in general, were more familiar with and had more positive opinions about psychiatric advance directives than other key informants. In particular, mental health key informants in the county that had psychiatric advance directive education were more familiar with and had more positive opinions about psychiatric advance directives than mental health informants in the county without the education. Implications for psychiatric advance directive education are discussed.
