The objective of this study is the investigation of the prognostic value of the 27 kD Bcl-2 protein in non-small cell lung cancer (NSCLC) patients. Currently, controversy surrounds the prognostic value of Bcl-2 in lung cancer. We argue that the method of Bcl-2 protein detection might have relevance, as multiple molecular forms of the protein are known to exist. In contrast to immunohistochemistry, the technique used by other investigators, we used Western blot, a more quantifiable method, to analyze Bcl-2 protein, and examined the prognostic value of the presence of Bcl-2 protein in uninvolved lung and in tumor tissues. We also compared adenocarcinoma and squamous cell carcinoma patients, and studied stage I versus stage II tumor patients.

We studied 91 lung cancer patients. The number of adenocarcinoma and squamous cell carcinoma patients was 45 and 36 respectively, 77% of the patients were of tumor stages I and II and 50% of the patients had relatively small (<4 cm) tumors. Our main findings—the presence of the 27 kD Bcl-2 protein expression in uninvolved lung tissue and in uninvolved lung tissue or tumor combined—was associated with shorter survival. The association was stronger in stage I patients, as 27 kD Bcl-2 expression in uninvolved lung tissue was associated with poor survival after adjusting for other covariates. We found no association between 27 kD Bcl-2 protein expression in the tumor and disease prognosis. In uninvolved lung tissue, 27 kD Bcl-2 protein expression was associated with shorter survival in adenocarcinoma patients, but not in squamous cell carcinoma patients. This is the first report on investigating Bcl-2 protein expression in uninvolved lung tissue as a prognostic marker for NSCLC. In particular, our observation is especially novel because comparison at the level of molecular identity (as 27kD Bcl-2) has not been previously reported in the literature. These findings are of potential clinical application, as identification of risk factors associated with poor survival could be informative for patient management.
