The first essay uses the Medicaid program, one of the largest public transfer programs in the United States, to examine the quality of care provided by managed care programs (HMOs) relative to that obtained by individuals covered by traditional fee-for-service insurance plans. Health economists and policy makers have long sought to answer this question; however, because selection into managed care plans is not thought to be random, identifying the effect of coverage on care has been difficult. I take advantage of a transition from a fee-for-service system to managed care within the California Medicaid program (Medi-Cal) to identify this effect. In addition to the switch from one type of coverage to the other, there was variation across geographic areas in the number of available health care plans and their administrative organization. These differences create variation in the level of competition within the healthcare market.

Using a unique longitudinal dataset and exploiting the variation in the timing of the implementation of Medicaid managed care plans across counties in California, my first essay examines the impact of different levels of competition on Medicaid recipients. Specifically, I estimate the effect that the state's change from traditional fee-for-service to various forms of managed care had on Medicaid enrollment, the adequacy of prenatal care, and birth outcomes. Contrary to expectations, I find that areas with the lowest level of managed care competition (fewest managed care plans) have the largest rates of enrollment and are appear to have provided the most adequate prenatal care; plans with a more moderate level of competition have done so with relatively smaller magnitude, while the most competitive regions exhibited the opposite results. My findings thus suggest that competition within the managed health care industry may not benefit Medicaid recipients.

The second essay is about time transfers (in the form of grandchild care) and monetary transfers to adult children, and the relationship between these transfers and elderly mothers' labor supply. In addition to the possibility of undertaking paid employment, many women also assume responsibility for providing caring for family members such as frail spouses, elderly parents and young grandchildren. The cost of this caregiving could manifest itself in reductions in leisure or reductions in labor supply. Conversely, mothers may aid their adult children by providing cash assistance, perhaps increasing hours worked in order to increase available financial resources. This paper investigates the relationship between grandchild care, financial transfers and paid employment of older women. I also examine, in detail, how these transfers vary with the gender and genetic relationship of the child.

The results show that older women are likely to assist their children by working more, either by increasing time spent in paid employment in order to provide monetary transfers to their children, or working in an informal capacity providing grandchild care themselves and maintaining their original level of employment. In addition, these women are much more likely to provide monetary transfers and time transfers in the form of grandchild care to daughters than to sons; they also tend to transfer more to biological children than to stepchildren.

The third essay is about the effect caring for a parent on the health of the caregiving daughter. As one of the most typical informal care in the United States, parent care will continue to increase as the population ages. Providing parent care is likely to be emotionally and physically burdensome, and could then affect the physical and emotional health of caregivers. However, due to the selection into caregiving roles, empirical research has not provided sufficiently convincing and consistent conclusions on this subject. Using a panel dataset and instrumental variable estimation, this paper examines the effect of parent care on caregiving daughters' emotional and physical health. My research shows that providing parent care negatively affects a daughter's self-perception of her overall health. It does not affect her overall perception of mental health as measured by the Center for Epidemiologic Studies Depression Scale (CESD), exhibiting mixed effects on the eight items composing this score. Caregiving also increases the incidence of being diagnosed with high blood pressure and/or arthritis. It does not affect other doctor-diagnosed conditions such as diabetes, lung problems, cancer and so forth.

Together these essays demonstrate the importance of public and private transfers in affecting various aspects of economic behaviors-health service use, labor market behaviors and health. Although provocative in many ways, these essays just turned the surface of these issues and much additional work will be needed to explore these issues fully.
