Operational failures arise when there is a difference between expected and observed outcome in terms of cost, quality, flexibility, reliability, and delivery speed. The sources of these operational failures can be frequent or rare. In cases of frequent operational failures, firms are in a position to continuously learn through organizational actions and response mechanisms. In contrast, because of a low probability of occurrence and thus a very small sample, it is challenging for firms to learn from their own rare operational failures or those experienced by other firms.

In line with this organizational challenge, the dissertation examines the association between rare operational failure and learning. The two major research questions addressed are: (1) At the industry level, what is the association between types of rare operational failures and population-level learning? How does industry structure influence population-level learning? (2) At the firm level, how does transfer of learning from rare operational failure occur between firms (vicarious learning)?

Drawing and building on extant research, this study argues that industry structure will determine population-level learning. Learning from rare operational failure will be higher for industries that have low concentration, high growth, and high dynamism. Moreover, learning will also be higher for industries that experienced variety in past operational failures. To investigate this association, I utilize a combination of regression techniques on a longitudinal dataset of operational failures spanning from 1994 to 2004. The results showed that heterogeneity of past operational failure influences learning. The research also contributes to the understanding of which industry conditions are important for learning.

At a firm level, the results from the case studies uncovered the factors that enable the process of vicarious learning from operational failure. Empirical evidence from six case studies led to propositions and a related research framework. The emerged categories suggested that the learning process is associated with the characteristics of (1) the operational failure (2) the firm experiencing the failure (3) the observing firm acquiring knowledge regarding the failure and (4) the industry in which the observing firm operates. Additionally, product and process complementarities and inter-firm linkages also enabled the learning process.
