The role of stress in the etiology of irritable bowel syndrome has been an ongoing matter of controversy in the prevailing literature. Nevertheless, stress management programs and cognitive therapy have been highly effective in treating patients with this disorder. To date, however, researchers have not addressed whether cognitive appraisals and maladaptive cognitions (such as cognitive rumination or worry) are temporally associated with symptom activity in irritable bowel syndrome. Furthermore, with one exception, researchers have not fully explored the association between daily stress and physical symptomatology over time and individual differences in the stress-IBS symptom relationship. Accordingly, the present study examined the magnitude, consistency, temporality and idiographic differences in the relationship between irritable bowel syndrome and the following constructs of interest: minor daily stress, daily cognitive appraisals, recurring intrusive negative thoughts and worry.

Twenty-six subjects with a diagnosis of irritable bowel syndrome monitored their minor stress, appraisal of the most bothersome even of the day, degree of recurring intrusive negative thoughts and daily worry, IBS symptom severity and medication intake. For the majority of subjects, there was compliance with a 28-day self-monitoring protocol. The indices of minor stress utilized were Frequency of Stress, Impact of Stress and Average Impact of Stress. The daily cognitive appraisal indices were Perceived Impact, Perceived Control, Perceived Predictability and Perceived Coping Outcome. An analogue measure of degree of recurring Intrusive Negative Thoughts and a modified version of the Penn State Worry Questionnaire were utilized to assess daily cognitive states. The Average Symptom Severity score (average daily rating of severity of eight IBS symptoms) provided the outcome indicator for symptom activity.

Results showed that the predictors, with the exception of two cognitive appraisal variables (Perceived Control and Perceived Predictability) had significant relationships with same day IBS symptom activity. The Intrusive Negative Thoughts predictor was the strongest, accounting for 4% of the variance in IBS symptom activity. Individual differences in the reliability and patterns of correlations over time were found. Overall, the results indicated that IBS symptom activity was best predicted by same day stress and cognitions. Implications of these findings for theory, further research and clinical practice are discussed.
