The quality of life of persons with communication disorders is especially dependent on their ability to function individually and socially following treatment interventions. There is, however, limited published research evidence examining the importance of health-related quality of life (HRQL) domains to individuals with communication disorders, their significant others, and/or their treating speech-language pathologists (SLPs). This study examined and compared item responses (perceptions of performance) and rankings of the importance of four HRQL domains (physical, communication, psychosocial, energy) and overall HRQL to persons with aphasia, their significant others, and their treating SLPs.

Twenty-four participants with mild, moderate, or severe aphasia, their significant others, and treating SLPs responded to the Stroke and Aphasia Quality of Life Scale-39 (SAQOL-39; Hilari, Byng, Lamping, & Smith, 2003) and Quality of Communication Life Scale (QCL; Paul, Frattali, Holland et al., 2004). In addition to the perception of performance responses for each scale, the respondents ranked each item by importance on a 5-point scale developed for this study. Severity distinguished the mild performance ratings on the SAQOL-39 for overall HRQL from the other severity groups, but was less effective in progressively distinguishing between groups as severity increased. A statistically significant difference for the performance ratings on the communication domain existed between the moderate severity group and the severe group, but not between the mild severity group compared to the other groups. The physical domain emerged as being more important to the person with aphasia than the communication, psychosocial, and energy domains. Results indicated no significant differences in ratings of importance for overall HRQL for respondents on the SAQOL-39 and QCL.

Results indicate further need for development of methods to assess health-related quality of life and domain importance to persons with mild, moderate and severe aphasia. Results also demonstrated that persons with severe aphasia can participate in health-related quality of life studies when instruments are accessible to them.
