The incidence of diabetes has reached epidemic proportions in the United States. Diabetes affects every major organ system of the human body; of particular focus here are the eye complications. The main purpose of this thesis is to better understand the early pathogenesis of diabetic retinal eye disease in adults and adolescents using multifocal electroretinograms (mfERG), optical coherence tomography (OCT) and digital fundus photography. Early detection of functional, structural and vascular changes will aid in the development of treatments that can prevent permanent damage or further loss of retinal function.

In chapter 3, the mfERG interocular comparisons did not show an advantage over traditional methods of between group comparisons in adults with diabetes but did show a significant test order effect when sequentially recording two eyes. A follow up study of the order effect was conducted.

Chapter 4 shows that only after a sufficient duration of diabetes and the appearance of early nonproliferative diabetic retinopathy, is there a spatial association between first order implicit time delays and second order abnormalities of fast adaptation.

The studies presented in Chapter 5 reveal early indications of focal retinal neuropathy in adolescents, in both type 1 and type 2 diabetes, with as little as one-year diabetes duration. Regional retinal thickness abnormalities are also reported in comparisons between adolescents with type 1, type 2 and no diabetes. Even more alarming, adolescents with type 2 diabetes were found to also present with significant venular enlargement, retinal thinning and elevated body mass index (BMI).

Collectively, these studies show neural retina dysfunction very early in the progression of diabetes in both adults and adolescents in the absence of vascular lesions. These are the first studies to examine local retinal function (neural) and structure in adolescents with diabetes. Based on these studies, longitudinal studies of the functional, structural and vascular changes in the adolescent diabetic population should be conducted.
