Periprosthetic osteolysis is a disease caused by the body's reaction to fine polyethylene wear debris shed from total hip replacements. The prevalence of this disease and the consequences of prosthetic loosening and failure make osteolysis the most significant long-term problem in total hip arthroplasties. The biologic process causing the disease is typically asymptomatic, but leads to a multifocal loss of the bone surrounding the hip implant along with the disappearance of its trabecular content.

520 $a The goal of this research is to investigate computerized radiographic methods to measure structural changes of the trabecular bone patterns over time. We hypothesize that temporal radiographic texture analysis (tRTA) has the ability to distinguish between osteolysis and normal cases at the current clinical detection time, and moreover, that tRTA may be able to help in the early detection of osteolysis. Earlier detection of osteolysis would help reduce the extreme complications of prosthetic loosening and fracture, which require complex revision surgeries.
520 $a In this research, we investigate various tRTA methods to measure texture changes over time, and assess their diagnostic performance. We investigate the performance of tRTA and radiographic texture analysis (RTA) for different groups based on age and activity level. We implement a model to evaluate the effect of various clinical covariates on the performance of RTA and tRTA, and use this model to assess the predictive power of these features. Finally, we perform a pre-clinical evaluation, by comparing feature performance for RTA and tRTA with observer ratings, in order to investigate their potential clinical usefulness for the early detection of periprosthetic osteolysis.
