Evaluating treatment efficacy for acquired apraxia of speech (AOS) in adults has been a challenge. To date only two qualitative reviews exist while attempts at quantitative reviews have been aborted. Disparity in treatment and evaluation methods, frank design limitations and a growing number of case studies replacing group studies have produced considerable obstacles to quantitative meta-analytic review. With the advent of new meta-analysis techniques it has become feasible to meet some of these challenges, particularly, the inclusion of single case studies.

The purpose of the present study was to conduct qualitative and quantitative analyses of published AOS treatment approaches. A qualitative analysis of 66 single- and multiple-subject studies of AOS suggested that articulatory/kinematic treatment and correct sounds or correct word productions were study variables that allowed a sufficiently homogeneous grouping for quantitative meta-analysis. This analysis further revealed that publication bias possibly skewed the results. With this caveat, eight group studies (5 using sounds correct and 3 using words correct as the dependent variable) and 11 single-subject studies (6 using sounds correct and 5 using words correct as the dependent variables) were included in two separate meta-analyses using META-5.3 program.

Meta-analyses were based upon r-values if they included group studies and Cohen's of d statistics if they aggregated pre-post test single case studies. Additionally, single-subject with time-series data were analyzed using an autoregressive integrated moving averages (ARIMA) procedure for both slope analysis and forecasting using ForecastX software. Forecasting data were compared to maintenance data and predictions of the efficacy of continued treatment were inferred.

The abovementioned analyses revealed treatment efficacy that was statistically significant whether it involved group or single case studies and irregardless of the dependent variable, i.e., sounds or words correct. In addition, forecasting analyses were significant for 13/20 studies indicating that continued treatment could have been efficacious. Overall, this study shows that AOS treatment is efficacious albeit that the speech repertoire where these gains are observed are limited. The forecast results in fact support the notion of broadening the scope of treatment as full treatment potential may not have been reached in a majority of single case studies.
