We found that plasma ferritin concentration alone provides a good approximation of total body iron reserves, estimated by the ratio of transferrin receptor/ferritin, in several age groups, based on high correlations (r > 0.95) and good sensitivity and specificity (> 0.90) among individuals with non-elevated C-reactive protein (n = 1189).

In fully breastfed infants (n=404) at 6 mo of age, we found that the significant predictors of low ferritin (20% overall) were immediate umbilical cord clamping (AOR 4.5), male sex (AOR 4.0) and birth weight between 2500 - 2999 g (AOR 2.3). The predictors of ID plus anemia (8% overall) were male sex (AOR 7.2), birth weight between 2500 - 2999 g (AOR 3.0), and weight gain since birth above the median (AOR 3.1).

We assessed acute effects on plasma iron, non-transferrin bound iron (NTBI) and isoprostane (a marker of oxidative stress) of an iron supplement given with or without rice porridge, using a cross-over design with a one-week washout period. Within 4 h after ingestion of 45 mg iron as FeSO4, plasma iron concentrations were significantly greater when given without a meal than with a meal. The increase in plasma iron was greater for subjects with low vs. high iron stores regardless of treatment. Plasma NTBI concentrations were marginally greater for subjects with high iron stores than for those with low iron stores, but they did not increase after ingestion of iron supplements. Plasma isoprostane concentrations did not differ by treatment or increase after ingestion of the iron supplement. The results suggest that giving iron with a meal limits iron uptake, particularly by subjects with high iron stores.
