520 $a Design. This was a pre/post intervention study. The study examined Shock Trauma Respiratory Intensive Care Unit (STRICU) progress note documentation before and after the implementation of an ECCPN.

To determine whether the physician Electronic Critical Care Progress Note (ECCPN) improved daily documentation compared to the handwritten progress note using the following criteria: (1) Patient problems; (2) Treatment/treatment plans; (3) Legibility; (4) Completeness including patient identifiers and physician signature; (5) Decrease in work duplication.
520 $a Methods. Patient medical problems and treatment plans were collected during morning rounds from July to October 2003 and from February to June 2006. The medical problems and treatment plans that were verified by the attending physician were used as the "Gold Standard" compared to the medical problems and treatment plans documented by housestaff in the patient's daily progress note before and after the implementation of the ECCPN.
520 $a Results. Documentation of medical problems improved by 12% (p <0.0001) and treatment plans improved by 15% (p < 0.0001) when the housestaff used the ECCPN. The process change implemented with the ECCPN also improved the discussion of relevant medical problems and treatment plans during ICU morning rounds. Use of the ECCPN increased legibility from 48% to 100%. Overall, mandatory progress note documentation (patient identifiers, physician signature, and date) showed a significant improvement (p< 0.0001).
