Introduction. The burden of cardiovascular disease (CVD) is of major public health concern, especially with respect to racial disparities which have been recognized for several years. In this dissertation we examine CVD mortality risk with regards to elevated blood pressure and diabetes to determine if there are significant black--white variations in strengths of associations while also considering various epidemiological methods in describing these associations.

Results. Our findings illustrate similar interpretations under the different one and two stage pooling techniques. However, ignoring sampling design in the analysis resulted in smaller standard error estimates. The risk of CVD mortality did not vary between blacks and whites with regards to increasing systolic blood pressure. Yet blacks had a four-fold larger proportion of values greater than 195 mmHg compared to whites (8.98% vs. 2.14%; p-value < 0.0001). Similarly, when assessing diabetes and hypertension levels, no racial variation was detected but blacks had significantly more individuals with diabetes and prevalence of stage 2 hypertension was nearly doubled among blacks compared to whites (20.5% vs 39.2%; p-value < 0.0001).

Conclusions. Long-term CVD mortality risk is similar for blacks and whites when the same risk factors are present. Thus the racial disparities that exist are mainly due to excess risk factors among blacks. More emphasis in preventing and controlling CVD risk factors in black populations are necessary.
