Thesis (D.N.P.)--University of Hawai'i at Manoa, 2024.
Trauma, an alarming event, or distressing experience, profoundly affects individuals' health and well-being, regardless of age, gender, or ethnicity. Traumatic events, such as violence and loss, alter physiological responses, necessitating trauma-informed care (TIC) in healthcare settings. This Doctor of Nursing Practice (DNP) project focused on delivering a TIC continuing education module to enhance Primary Care Provider (PCP) competency and care quality at the Primary Care Clinic of Hawai{CA}{BB}i (PCCH). The project addressed the high prevalence of trauma in communities, particularly among Filipino Americans, and the elevated mental health needs in the clinic's catchment areas. A critical need for TIC training in primary care exists, especially in regions with high mental health needs (i.e., Honolulu County). By equipping PCPs with TIC competencies, the project endeavored to enhance patient care quality, particularly for those with trauma histories. A Zoom-based TIC course, recorded for accessibility, engaged PCPs, emphasizing trauma understanding, TIC implementation, and post-trauma growth. Data collection involved pre- and post-training surveys to assess knowledge, beliefs, and practices. The training aimed to improve PCPs' knowledge, beliefs, and practices concerning TIC. Post-training evaluations gauged the effectiveness of the modules in enhancing PCP competencies and shaping clinical practices. Results from the post-training survey indicated significant improvements among participants who completed the assessment. These improvements included heightened knowledge in identifying trauma symptoms, increased confidence in providing TIC, and a positive shift in beliefs toward the importance and effectiveness of trauma-informed approaches. Qualitative feedback underscored the benefits of TIC in strengthening provider-patient relationships and improving patient outcomes through timely information delivery. Moreover, it{A0}highlighted the practicality and efficacy of implementing TIC as a strategy for enhancing practice. These findings suggested that the educational training session successfully yielded positive changes in knowledge, beliefs, and practices related to TIC among participating PCPs. Despite limitations in post-training survey completion, the results affirmed the effectiveness of targeted educational interventions in strengthening PCPs' capacity to deliver TIC. Successful implementation of TIC education can lead to improved patient-provider relationships, increased trauma identification, and more effective trauma-informed interventions. Continued evaluation and refinement of TIC training programs were recommended to ensure ongoing quality improvement in healthcare delivery, especially in regions with elevated mental health needs.
