Hypertension (HTN) is a serious global public health issue that affects as many as 1.3 billion people worldwide and 116 million Americans. Nearly 1 out of 2 adults in the U.S. (47% of the total population) are impacted by this chronic condition (CDC, 2023). It is a significant burden and is considered the leading preventable risk factor for cardiovascular disease with as many as 1.6 million associated deaths each year. Uncontrolled HTN contributes to complications of heart attacks, strokes, and kidney disease with associated poor outcomes. According to data from the CDC (2023), only about 1 in 4 adults with hypertension have this chronic condition under good control. The improved management of hypertension requires various strategies to impact the overall outcomes, including medication and treatment adherence, and empowering individuals to engage in essential self-care behaviors. The purpose of this quality improvement DNP project (QIP) was to evaluate a telehealth Chronic Care Management (CCM) program utilizing Self-Management Support (SMS) tools to inspire, engage, motivate, educate, and empower HTN patients within Farrington Cares (FC). Participants with a HTN diagnosis (ICD-10 I10) participated in two telehealth health coaching sessions over an 8-week intervention period. The effectiveness of the SMS health coaching sessions was evaluated by pre-assessment and post-assessment utilizing the John Hopkins Hypertension Self Care Profile (HBP-SCP) tool. This tool measured self-care behavior, motivation, and self-efficacy of hypertension. The collected data was analyzed using SPSS software. Results demonstrated a statistically significant improvement in post-implementation scores. In conclusion, the SMS health coaching sessions demonstrated improved self-care habits and self-efficacy over the 8-week intervention period.
