Compassion is a multifaceted phenomenon that includes awareness, emotional response, pro-social behaviors, and an intrapersonal sense of benevolence. In healthcare, compassion is arising as a potential practice to mitigate or relieve the adverse effects of compassion fatigue and burnout, which impact the lives of over 50% of physicians and upwards of 40%-80% of health care professionals and students (Reith, 2018). The aim of this study is to understand how health care providers foster their experience of compassion amid the unfavorable conditions of their profession through the qualitative lens of grounded theory.I employed Straussian grounded theory methodology to explore the intrapersonal processes of generating compassion. Straussian grounded theory aligns with the pragmatic philosophy that beliefs lead to actions and actions lead to consequences (Corbin & Strauss, 2015). Furthermore, the consequences then lead to revised beliefs, new actions, and further consequences. This aspect of the human experience is reflected in the cyclical and continuous nature of data collection and analysis specific to grounded theory. Concerning this study, the truth of one's compassion is not the issue I sought to explore, but how one acts to foster that experience. Therefore, using a method with underlying pragmatic principles benefited the overall concept of the study. I collected data via semi-structured interviews of health care providers who have been working directly with patients. Twenty-seven providers were recruited through purposive and snowball sampling, met all inclusion criteria, and were interviewed for the study. A total of 379 pages of data were coded using open, axial, and selective coding methods including constant comparison and reflective coding matrices. Through this process, four core categories emerged: (a) Engaging With Self, (b) Engaging With Heart, (c) Engaging With the System, and (d) Engaging With Purpose.Gaining insight into the processes, practices, and mind-states necessary to foster compassion illuminated ways to support the physical and mental health of health care providers. Compassion training programs are still in their infancy, and it is my hope that the results of this study will advance the design and application of compassion training to mitigate the adverse effects of compassion fatigue and burnout.
