Rehospitalization among adults with congestive heart failure causes high healthcare costs, thus necessitating interventions to reduce rehospitalization rates. This study focused on weekly telephone call intervention for medication reminders to decrease rehospitalization among adults with congestive heart failure within eight weeks. The study was a quantitative quasi-experimental study from a convenience sample of 30 participants. The participants were retrieved from a population of adults from all racial groups aged 18 to 65 with congestive heart failure from a facility in Bronx, New York. The rehospitalization incidences of the sample were investigated in the 30 days before and after the intervention to determine the pre-and post-intervention rehospitalization outcomes. An analysis of the study's findings revealed that weekly telephone calls for medication reminders intervention yielded a statistically significant reduction in the rehospitalization of participants after the intervention (t (26) = 7.04, p < .001). Clinically, the findings support that weekly telephone calls for medication reminders should be used among adults with congestive heart failure to reduce rehospitalization rates and promote well-being.
