This thesis examined healthcare associated infections (HAIs) and HAIs caused by carbapenem-resistant (CR) Gram-negative bacteria (GNB) in Singapore. In a nationwide HAI point prevalence survey, one in nine adult inpatients admitted to Singapore hospitals had at least 1 HAI(s). Compared to patients without HAIs, patients with HAIs had increased length of stay and mortality risks. Risk factors for CR-GNB HAIs included prior carbapenem exposure, prior fluoroquinolone exposure and presence of drains. Compared to patients with carbapenem-susceptible GNB HAIs, patients with CR-GNB HAIs had significantly longer time to discharge. This translated to a markedly higher costs to hospitals. In patients with CR-GNB HAI patients who received different polymyxin B-based treatment, those who received treatment guided in vitro antibiotic combination testing had improved clinical outcomes. In summary, HAIs and CR-GNB HAIs are important public health problems in Singapore. A multi-pronged approach is necessary to reduce HAIs and combat carbapenem resistance among GNB.
