Although healthcare policy and legislation support Advance Care Planning (ACP), participation rates in the United States remain low. Failure to engage community members in ACP discussions may be attributed to lack of knowledge and social endorsement of the benefits, and process of ACP, leading to poor perceived ACP self-efficacy. Failure to participate in ACP may result in lack of clear wishes for end-of-life care and symptom management, causing needless suffering, and futile treatment. The purpose of the study was to determine if a nurse-led information program improved participants' perceived self-efficacy to discuss ACP with their families and healthcare providers. Participants were recruited from church congregations in five Northern Kentucky counties. The Advance Care Planning Engagement Survey was administered prior to, and following an educational presentation on the Five Wishes? tool. Analysis using a paired t-test indicated significant improvement (p <0.000) in perceived self-efficacy to engage in ACP.
