Introduction:Occupational therapists evaluate occupational performance by older adults with cognitive impairments. Previous studies of the methods used by occupational therapists to evaluate this population suggest there is a lack of understanding of clinical practice in this domain, as well as a lack of tools. Given the importance of occupational therapy interventions in supporting the well-being of these individuals, we aim to understand the process that occupational therapists use to evaluate older adults with cognitive impairments, identify the knowledge and tools occupational therapists need to support them in their evaluations, and identify their preferred strategies to acquire new knowledge.Objectives:This study had three objectives : 1. To understand how occupational therapists evaluate cognitively impaired older adults and the reason behind their clinical choices ; 2. To understand the knowledge and tools needs of occupational therapists in this area of practice ; and 3. To take preliminary steps to identify knowledge transfer strategies that will increase uptake of research evidence into practice.Methods:A descriptive qualitative study with a deductive-inductive approach was used. Focus groups were conducted with occupational therapists who had at least one-year experience working with older adults with cognitive impairments. Recruitment of participants was discontinued when no new content emerged from the groups. Key interview questions were constructed to obtain information responding to the objectives of our study. Thematic content was analyzed using the Miles et al. (2014) approach for qualitative data analysis. A deductive approach was used for first cycle coding, allowing us to sort data into main content areas. An inductive approach was used to probe each content area, allowing us to further refine our analysis and create sub-themes. Sub-themes were then combined into overarching themes. The analysed data was presented in network and matrix displays to show how the themes responded to our research objectives.Results:Data saturation was reached upon interviewing sixteen occupational therapists (n = 4 focus groups). Each participated in two focus group meetings. Three themes emerged reponding to the first objective: 1) gather information from the individual and others involved to plan the evaluation ; 2) gather information from non-standardized observation and standardized testing to evaluate occupational performance and safety ; and 3) apply a clinical reasoning process to predict occupational capacity. Three themes responded to the second objective, describing the knowledge and tools needed to support participants in their evaluations. This included 1) knowledge related to cognition and occupational performance ; 2) knowledge related to legal capacity and protection regimes ; and 3) tools to evaluate the impact of cognitive function on occupational performance. Two themes emerged for the third objective, which described the preferred ways for participants to acquire new knowledge including: 1) formal interactions with experts ; and 2) informal interactions with students, peers and other professionals.Conclusion:Our findings indicate that there is a need for improvement within the evaluation process in order for occupational therapists to optimize their capacity to understand the impacts of cognitive impairments on occupational performance with more confidence and rigor. Clinicians have an important need for additional knowledge and tools to support them in their evaluations. The continued development of psychometric properties for existing tools is needed as well as collaboration between clinicians, management and research experts to move existing tools into practice. Future research should explore the needs of occupational therapists other than those related to knowledge and tools.

520 $a Introduction:Les ergotherapeutes evaluent frequemment le rendement occupationnel des personnes agees presentant des troubles cognitifs. Des etudes anterieures sur les methodes utilisees par les ergotherapeutes pour evaluer cette population suggerent qu'il y a un manque de comprehension de la pratique clinique dans ce domaine, ainsi qu'un manque d'outils. Nous visions avec ce projet a comprendre le processus que les ergotherapeutes utilisent pour evaluer cette population, a identifier les connaissances et les outils dont les ergotherapeutes ont besoin pour soutenir leurs evaluations, et a identifier leurs strategies privilegiees pour acquerir et mettre en pratique de nouvelles connaissances.Objectifs :L'etude avait trois objectifs : 1. Comprendre comment les ergotherapeutes evaluent les personnes agees presentant de troubles cognitifs et la raison de leurs choix cliniques ; 2. Comprendre les besoins en connaissances et outils des ergotherapeutes dans ce domaine de pratique ; et 3. Identifier les strategies de transfert des connaissances a privilegier pour faciliter la mise en pratique des donnees probantes dans le domaine.Methodes:Une etude qualitative descriptive avec une approche deductive-inductive a ete utilisee. Des groupes de discussion ont ete organises avec des ergotherapeutes qui avaient d'experience de travail avec des personnes agees presentant des troubles cognitifs. Le recrutement a ete arrete lorsqu'aucun nouveau contenu n'est apparu dans les groupes de discussion. Le contenu a ete analyse a l'aide de l'approche de Miles, Huberman et Saldana (2014). Une approche deductive a ete utilisee pour le premier cycle de codage. Une approche inductive a ete utilisee pour une exploration plus approfondie de chaque concept, ce qui nous a permis de raffiner notre analyse et de creer des sous-themes, qui ont ete combines en themes. Les donnees analysees ont ete presentees a l'aide de reseaux de concepts et de matrices, afin de presenter les relations entre les themes liees a nos objectifs.Resultats:La saturation des donnees a ete atteinte en interrogeant seize ergotherapeutes (n = 4 groupes de discussion). Chaque ergotherapeute ont participe a deux reunions du groupe. Trois themes ont emerge en reponse au premier objectif : 1) recueillir des informations aupres de l'individu et des autres personnes impliquees pour planifier l'evaluation ; 2) recueillir des informations a partir d'observations non standardisees et de tests standardises pour evaluer le rendement occupationnel et la securite ; et 3) appliquer un processus de raisonnement clinique pour predire le rendement occupationnel. Trois themes ont emerges en reponse au deuxieme objectif : 1) le besoin de connaissances liees a la cognition et au rendement occupationnel et 2) le besoin de connaissances liees a la capacite juridique et aux regimes de protection ; et 3) des outils pour soutenir l'evaluation de l'impact des fonctions cognitives sur le rendement occupationnel. Deux themes ont emerge pour le troisieme objectif : 1) des interactions avec des experts ; et 2) des interactions avec des etudiants, des pairs et d'autres professionnels.Conclusion:Nos resultats indiquent qu'il y a un besoin d'une amelioration des processus d'evaluation afin que les ergotherapeutes optimisent leur capacite a comprendre les impacts des troubles cognitifs sur le rendement occupationnel avec plus de confiance et de rigueur. Les cliniciens ont des besoins importants de connaissances et d'outils supplementaires pour les soutenir dans ces evaluations. Le developpement continu des proprietes psychometriques des outils existants est necessaire. Une collaboration etroite entre les cliniciens, les gestionnaires et les chercheurs pour mettre en pratique des outils existantes est egalement soulevee. Des recherches futures devraient explorer les besoins des ergotherapeutes pour ameliorer leur pratique dans le domaine, en plus de celles liees aux connaissances et aux outils, et explorer davantage les obstacles et facilitateurs a l'amelioration de leur pratique.
