Novelist, Eudora Welty has famously quoted, "The first act of insight is to throw away the labels." Language is a system utilized for conveying understanding and communication. For humans to develop understanding, classifications, labels, categories, and so forth are often used. My dissertation addressed how meaning and understanding of self was determined separate from complex diagnoses, specifically with a client diagnosed with dissociative identity disorder (DID). Much of the current research emphasizes modern psychotherapies as the preferred and most effective forms of treatment when working with clients diagnosed with DID. However, there is minimal research about the utility of post-modern therapies when working with complex diagnoses such as DID. The purpose of my study was to gain a deeper understanding of how a post-modern therapy, Solution-Focused Therapy, aided in helping a client with a complex diagnosis facilitate desired change. A single Solution-Focused Therapy session of an ongoing case was video recorded and transcribed. Conversational analysis was employed to conduct a detailed, minute examination of the therapeutic interaction. I thoroughly examined the spoken exchanges between the client and therapists second-by-second to highlight the client's transition from a negative internalized view to a deeper understanding of who they are. Six conversational building blocks emerged as the analysis progressed: 1) Humorous Problem-Free Talk, 2) Evaluating the Effectiveness of the Intersession Task, 3) Reframing Problem Talk, 4) Exceptions, 5) Bridging Statement and Metaphor, 6) Homework/Task. I have highlighted several exemplars and interpretations from each conversational building block. My study demonstrated how to collaboratively work with clients presenting with complex diagnoses through the usage of Solution-Focused Therapy interventions.
