BACKGROUND: Studies highlight the need for comprehensive community-based services for children with special health care needs (CSHCN) and their families to address the social and environmental barriers that impede positive life course trajectories; however, families in Oklahoma consistently report these service to be scarce. This dissertation's focus is to support the development of an effective health care system for CSHCN that promotes inclusive service delivery and prevents adverse life outcomes. METHODS: A scoping review and two original studies were conducted. The scoping review examines the evidence of measurable facilitators to successful health care transition programming in the United States. The first original study uses a phenomenological approach to understand the health care experiences of young adults with special health care needs, family caregivers, and health care providers. A team of researchers and family caregivers conducted a series of virtual focus groups and interviews to illuminate contextual considerations for health care transition planning in Oklahoma. The second study examined the relationship between CSHCN's sociodemographic characteristics and their receipt of quality health care services in Oklahoma. RESULTS: The scoping review highlighted transition readiness measurement, electronic health record (EHR) integration, and family-centered care as critical facilitators to health care transition programming. Findings from the second and third studies indicated that care coordination, system navigation, and follow-up were structural barriers specific to Oklahoma's health care transition planning. DISCUSSION/CONCLUSION: The three studies offer critical insight into the structural mechanisms needed to support the delivery of timely, well-planned, and family-centered health care transition services for CSHCN and their families. The dissertation offers policy recommendations for Oklahoma programs to improve family-centered health care transition services for CSHCN and families.
