More than five million people worldwide have a form of lupus and systemic lupus erythematosus accounts for seventy percent of the estimated 16,000 new annual cases in the United States. Individuals who experience disease complications include premature cardiovascular disease, fatigue, and musculoskeletal conditions. Resulting from these complications can include an increase in physical inactivity, leading to an increased risk of morbidity and mortality. Physical exercise has demonstrated to be safe and does not cause adverse effects on disease activity in individuals with systemic lupus erythematosus. Aerobic capacity has been shown to be reduced in this population, however, there is limited evidence comparing the effects of aerobic exercise to resistance training on quality of life and fatigue. CINAHL, PubMed, and ScienceDirect were databases searched using variations of the following search terms: "systemic lupus erythematosus", "aerobic exercise", "resistance training", "quality of life" and "fatigue". Eight studies eligible for this meta-analysis met the inclusion criteria and exclusion criteria. The studies measured either one of both of the following outcome measures: physical function subscale of quality of life and fatigue. Physical function presented with homogenous data with a grand effect size of 0.38. Fatigue also demonstrated homogeneity with a grand effect size of -0.33. Aerobic exercise demonstrated greater effectiveness towards improving physical function. Alternatively, although fatigue trended towards resistance training, there was insufficient findings to definitively show it had a greater effect compared to aerobic exercise. Future research is needed to determine the long-term effects of both exercise interventions and appropriate dosage protocols regarding resistance training.
