Appointment non-adherence in mental health clinics is a phenomenon that happens frequently and can result in financial difficulties for clinic owners and delays for patients desiring an expedient appointment. Extensive studies have been conducted to understand cancellations, no-shows, and other varieties of appointment non-adherence and strategies that may effectively reduce the high rates of schedule non-adherence. Based on the evidence, telehealth was identified as a viable option to address appointment non-adherence (McNeal et al., 2019). Due to restrictions implemented during the COVID-19 pandemic, telehealth technology was utilized for all appointments during a three-month period; yet significantly, appointment non-adherence rates remained nearly the same. Kroll et al. (2016) reported allowing patients to attended open appointments reduced non-adherence rates and improved patient outcomes. A pilot project was implemented to trial an innovative approach to reduce telehealth appointment non-adherence. The pilot project was implemented at a private mental health clinic. All existing patients were offered the opportunity to engage in the pilot project. The innovative pilot project entailed utilizing telehealth coupled with an open appointment format. This option was offered one day per week through the mental health clinic, negating the need for a scheduled appointment. Qualifying patients voluntarily chose to participate in an open appointment during the designated day of the week using a secure video conference technology. During the three-month open telehealth appointment phase, non-adherence rates decreased from 16% to 14%. The pandemic likely produced innumerable variables affecting project outcomes.
