Intensive Care Unit (ICU) nurses, exposed to more critical incidents, more end-of-life issues, and more complex procedures, face higher rates of compassion fatigue (CF) and lower rates of compassion satisfaction (CS) than nurses in traditional settings such as in-patient general units (Al-Majid et al., 2018; Boyle, 2015; Hinderer et al., 2014; Sacco et al., 2015). In addition, research suggests ICU nurses have less than optimal health promotion practices that could buffer against workplace stressors and prevent compassion fatigue (Letvak, 2013; McElligott et al., 2009). Statement of the Problem: Inadequate health promotion (HP) practices among ICU nurses result in poor physical and psychological health outcomes (CF) affecting personal well-being (CS) and indirectly influencing nurses professionally in their delivery of patient care and lost work hours. This research focused on two approaches intended to mitigate CF: self-compassion and health promotion (HP) testing a model of self-compassion, health promotion, compassion fatigue, and compassion satisfaction in ICU nurses. Testing the model required exploration of the pathways between personal factors (age, years as a nurse, education level, and sex), self-compassion, HP, CF, and CS as well as the mediational effect of HP on the relationship between personal factors and CF or CS. Methods: This IRB approved study utilized a cross-sectional design with convenience sampling to recruit 200 registered nurses working in select ICUs from a northeastern U.S area health system. Three instruments and a demographic form provided the data collection tools. Data analysis included descriptive and bivariate statistical analyses and structural equation modeling including an estimation of the measurement model via confirmatory factor analysis, testing of model constructs for convergent and discriminant validity, and testing the structural model and theorized causal relationships among constructs. Results: Aim 1: Age (β = .17, p < .05) and years spent as an RN (β = .16, p < .05) demonstrated a direct effect on self-compassion. Aim 2: Only self-compassion predicted health promoting behaviors, β = .44, p < .001. Aim 3: Self-compassion negatively predicted compassion fatigue, β = -.46, p < .001 and sex negatively predicted compassion fatigue, β = -.11, p < .05. Aim 4: None of the factors assessed showed a direct effect on CS. Aim 5: Health promoting behaviors negatively predicted compassion fatigue, β = -.24, p < .001 Aim 6: Health promoting behaviors positively predicted compassion satisfaction, β = .70, p < .001. Aim 7: Health promoting behaviors significantly mediated the relationship between self-compassion and compassion fatigue (β = -.46, p < .001). Aim 8: The model failed to confirm that health-promoting behaviors significantly mediated the relationship between self-compassion and compassion satisfaction. Summary of Conclusions: The findings related to these eight studies aims described significant interactions between self-compassion, HP, and compassion fatigue and compassion satisfaction. Prior research supports many of these study findings. However, this study also represents a novel investigation of the mediating role of HP in the relationship between self-compassion and CF and CS. These findings highlight the importance of both self-compassion and HP in combatting compassion fatigue and promoting compassion satisfaction among ICU nurses.
