Women with a BRCA (BReast CAncer) gene mutation carry a significantly increased lifetime risk of breast, ovarian, and other cancers compared to the general population. The United States Preventative Services Task Force (USPSTF) has issued a Grade B recommendation that primary care providers identify women at increased risk of having a BRCA mutation based on ancestry, family history, and personal history, through using a validated risk assessment tool. However, studies have identified gaps in collection of family cancer history and systematic cancer risk assessment in primary care. The purpose of this quality improvement project was to determine the impact of utilization of the Referral Screening Tool, a validated BRCA risk assessment tool, on the number of genetic counseling referrals made by providers at the project clinic in a moderately-sized city in Kansas. Project aims were to 1) determine the number of genetic counseling referrals made at the project clinic over the two-month period one year prior to the intervention, 2) implement the Referral Screening Tool (RST) to women aged 18 years and older who presented to the clinic for a physical exam or well-woman exam over a two-month intervention period, and 3) determine the number of genetic counseling referrals made during the two-month intervention period. Patients with a positive screening were to receive two Centers for Disease Control and Prevention (CDC) resources by their provider. Methodology included reviews of the electronic health record. The 86 women who completed the Referral Screening Tool had a mean age of 49.2 years. The screening positivity rate was 9.3% (n = 8). Electronic health record reviews revealed that use of the RST did not result in an equal or greater number of genetic counseling referrals compared to the two-month pre-intervention period used for comparison. However, the project resulted in the identification of two women found to be at increased risk who are considering genetic counseling and otherwise may not have learned of their increased risk status.
