In complex techniques like craniospinal irradiation for pediatric patients, lack of standardization may lead to an increased risk of side effects post radiation therapy. Craniospinal irradiation is a challenging and infrequent type of treatment for patients with malignancies in the brain such as medulloblastomas because it has a significant high possibility of metastasis throughout the spinal cord. Due to the small number of patients with this pathology, in addition to the difficulty of prioritizing dose constraints to target and organs at risk, planners and physicians regularly refer to previously-treated plans for guidance.This particular manner of planning new patients receiving craniospinal irradiation can be greatly improved by implementing a dose-volume histogram registry able to manipulate previous patient data to provide updated constraints regarding organs at risk.Our work shows that the treatment of new patients receiving craniospinal irradiation may be improved by using planning constraints derived from previous patient data in a DVHR.

Le manque de standardisation dans les techniques complexes comme l'irradiation craniospinale pour les patients pediatriques, peut entrainer un risque accru d'effets secondaires apres la radiotherapie. Ce type de traitement represente un challenge pour les patients atteints de tumeurs malignes dans le cerveau avec une haute possibilite de metastases dans la moelle epiniere comme par exemple les medulloblastomes. En raison du nombre peu eleve de patients atteints de cette pathologie, en plus de la difficulte de prioriser les contraintes de dose a cibler et les organes a risque, les planificateurs et les medecins se referent regulierement aux plans precedemment traites.Cette maniere de planifier de nouveaux patients recevant une irradiation craniospinale peut etre grandement amelioree en utilisant un registre d'histogramme dose-volume (DVH) capable de manipuler les donnees des patients precedents pour fournir des contraintes mises a jour concernant les organes a risque.Nos travaux montrent que le traitement des nouveaux patients recevant de l'irradiation cerebrospinale peut etre ameliore en utilisant des contraintes de planification derivees des donnees precedentes des patients dans un DVHR.
