The problem investigated by this study was the need for knowledge of family caregivers' experiences of hospital readmissions because of the recurrent problems with the transitional care process (Arbaje et al., 2014). The purpose of this qualitative phenomenological interpretive design was to explore lived experiences of these family caregivers. The theoretical framework aligned for this study was called the transtheoretical Model to promote health behavioral change in both the family caregiver and older adult. The research question was: what are the lived experiences of family caregivers concerning the transitional care process of older people with complex health conditions? The researcher contacted two private organizations who were approved by the Institutional Review Board. Data was collected in May and June of 2020 from fourteen participants in Mississippi. Data was obtained responsibly and analyzed accurately with the NVivo software. The results revealed the lived experiences of family caregivers and their essential needs in the transitional care process such as provider involvement, communication, and interaction about their older adult's health process. There were ten themes that resulted from this study that had implications for research and practice. This study might have served to buffer that family caregivers and their older adults experience into the healthcare system are impacted by changes during the transitional care process which entails hospital readmissions, adverse mishaps, and high mortality rates. Future research suggested goals were to explore specific behaviors or needs of a family caregiver, identify specific education for healthcare providers regarding effective behaviors for family caregiver interaction, and provide a mixed method approach to identify quantity of family caregivers impacted during the transitional care process cross cultural relations.
